What is a sliding fee and how do I apply?
 
 

As a Community Health Center, Health Access Network offers reduced rates for patients that are at the federal poverty level, but less than two hundred percent (200%), in accordance with the established sliding fee schedule. Patients who qualify at zero percent (0%) are charged the minimum payment of $5.00 for doctor visits and $20.00 for dental visits. At the higher end of the scale patients pay as much as 50 percent (50%) of the visit. Durable supplies are not included in this service, but the patient will be billed at our cost.

In order to qualify, the patient must fill out a Sliding Fee Application and provide current proof of income.  We accept tax returns, copies of bank statements, checks and award letters from SSI and the state.  If a patient is eligible for sliding fee, and upon review, is expected to also be eligible for MaineCare coverage, an interterm sliding fee discount will be authorized. The patient will be notified in writing and will be given 30 days to contact the billing department or the sliding fee expert to begin the process of MaineCare eligibility.
